Make a simple family care plan
For families who want to support an older parent without everyone assuming someone else will do it. Work through this plan with the older person. Print it or copy it into your own family notes. Nothing you write on the paper is sent to us.
Start with one routine. The older person chooses what to share. If you are worried, make contact directly; a missing app alert never proves that everything is fine.
1. Who does what?
Choose one main contact and one backup. Decide who can help if a phone has run out of power or an app stops working.
- Main contact: ____________________________
- Backup contact: ____________________________
- Someone nearby: ____________________________
2. Which routine would actually help?
- A daily check-in at a time that suits the older person.
- A shared overview of upcoming appointments.
- Medication reminders and a record of doses marked taken.
- Help getting home during an agreed trip.
Our first routine: _____________________________________________
3. What if we cannot reach them?
- Agree when the main contact calls directly.
- Agree when the backup or someone nearby is contacted.
- If you suspect immediate danger, contact local emergency services. Do not wait for an app notification.
Our agreement: ______________________________________________
4. What would the older person like to share?
Discuss check-ins, appointments, medication and location separately. Share only what is useful. Location can be stale or unavailable if the phone is not carried, charged, connected and permitted to share.
We agree to share: ___________________________________________
5. When will we review this?
Try the routine for one week. Ask whether it brings reassurance or just more notifications. Change the plan before adding more.
Review date: ________________________________________________
Keep a completed plan private. Never put health information or phone numbers in public comments.
